In addition to face to face consultations, Age Right offers support either by phone or email.
Repeat prescriptions can also be requested via the form below.
Phone Support
To book phone support for an existing patient or to discuss the needs of a family member with an Age Right Geriatrician, please complete the form below and make the required payment. We will get in touch with you to provide some date and time options.
The options are:
- 15 minute time slot $50
- 30 minute time slot$100
Email Support
For email advice from an Age Right Geriatrician for an existing patient, please complete the form below with a full description of the support needed and make the required payment.
Email advice will be provided within two business days. If this is an emergency then please contact an ambulance/ hospital/ GP surgery.
The cost is $50 per email or $100 for a fortnight support via email.
Repeat Prescription Request
For a repeat of your prescription/s, please fill in the form below and include the name of the medication and dosage. The cost is $15. You can choose to receive your script through the following means:
- Direct to your chemist
- By post
Please allow two business days to process your request.
Payment Options
Mastercard or Visa is accepted as a means of payment. Please be aware that your credit card details will not be stored on file.
